Herdegen and Klé Laboratory  Klé Laboratory, a Lyon based company, has been acting for 20 years as a major player in the bedsores prevention, developing, manufacturing and selling viscoelastic gel and memory foam cushions, mattresses and pillows. Klé has been offering a wide range of products for the prevention of bedsores, bringing comfort for patients in nursing homes, hospitals and at home. As part of its international growth strategy, Herdegen, took over the Klé company in December 2012. Thus, we acquired new know-hows in the foam and gel production, enabling us to offer new product lines to our worldwide sales network. Bedsores Pressure ulcers, also known as decubitus ulcers or bedsores, are localized injuries to the skin and/or underlying tissue usually over a bony prominence, as a result of pressure, or pressure in combination with shear and/or friction. Most commonly this will be the sacrum, coccyx, heels or the hips, but other sites such as the elbows, knees, ankles or the back of the cranium can be affected. Made in our laboratory, tested and approved by the French Ministry of Health, our products reduce these pressures, limit in a significant way bedsores and enable their healing.

Pressure ulcers are likely caused by three different tissue forces: 1. Pressure, or the compression of tissues and/or destruction of muscle cells. In most cases, this compression is caused by the force of bone against a surface, as when a patient remains in a single decubitus position for a lengthy period. 2. Shearing, a force created when the skin of a patient stays in one place as the deep fascia and skeletal muscle slide down with gravity, can also cause the pinching off of blood vessels which may lead to ischemia and tissue necrosis. 3. Microclimate, the temperature and moisture of the skin in contact with the surface of the bed or wheelchair. Moisture on the skin causes the skin to lose the dry outer layer and reduces the tolerance of the skin for pressure and shear.

Risk factors Impaired mobility - People with decreased mobility often have difficulty moving to relieve pressure. They may be unable to reposition themselves in a chair, increasing pressure over prolonged periods of time, or may slump, causing a shearing effect (paralysis, recovery after surgery, coma, wheelchairs’ users..). Other factors that increase the risk of pressure sores include: Age, lack of sensory perception, weight loss, poor nutrition and hydration, urinary or fecal incontinence, excess moisture or dryness, medical conditions affecting circulation, smoking, decreased mental awareness, muscle spasms.

Predicting pressure sore risks The Braden Scale for Predicting Pressure Ulcer Risk, is a tool that was developed in 1987 by Barbara Braden and Nancy Bergstrom. The purpose of the scale is to help health professionals, especially nurses, assess a patient's risk of developing a pressure ulcer. The Norton Scoring system created in England in 1962, has been the first pressure sore risk evaluation scale to be created, and for this it is now criticized in the wake of the results of modern research. Its ease of use, however, makes it still widely used today.

Advanced technology to meet the medical requirements The polyurethan : Polyurethane (PUR and PU) is a polymer composed of a chain of organic units joined by carbamate (urethane) links. Polyurethane polymers are formed by reacting an isocyanate with a polyol. Polyurethanes are used in many various fields : the manufacture of flexible, high-resilience foam seating; rigid foam insulation panels; durable elastomeric wheels and tires; high performance adhesives; carpet underlay; skateboard wheels.

Viscoelastic or «Memory» foam : Memory foam is polyurethane with additional chemicals increasing its viscosity and density. It is often referred to as «viscoelastic» polyurethane foam. A memory foam is pressure-and thermosensitive and molds quickly to the shape of a body pressing against it, returning to its original shape once the pressure is removed.

CosyPUR© Visco from BASF : The Memory foam we are using was developped by BASF. We are able to mold densities from 45 kg/ m³ up to 250 kg/m³. Basically highly elastic foam is viscoelastic due to it‘s special micro-perforated cell structure. Viscoelasticity is adjustable by variation of cell perforation,i.e. a more perforated cell structure is less viscoelastic. Pleasant touch due to very fine cell structure. Low emission and low odor foams. Special formulations, for example flame-retardant foams are available.

The gel : Mix between polyol and isocyanate molded in low pressure.The impact absorption property can be indicated by rebound property of the material. The major advantage of our gel is that unlike other water or silicone based gels, it doesn’t lose its shape, harden, break or age. This is due to the simple fact that it doesn’t contain plasticizers. PU gels are not just applied to medical products, but can also be found in padding for bicycle saddles for its comfortable and shock absorbing nature.

Honeycomb or 3D cover : Our high quality honeycomb or 3 Dimensions cover features a good air circulation, dissipates heat and moisture to keep the surface clean and dry. This cover is made of a special knitted cotton. With zipper. Machine washable.

Urethane cover : Made of waterproof urethane, with a zipper and 2 handles to carry it.

Foam & Gel Seat Cushions The Herdegen line of seat cushions ranges from Gel to Foam cushions designed to give support to patients whilst in wheelchairs and/or day chairs. Our cushions ranges from Low Risk to High Risk. We use only the best quality raw materials to manufacture our cushions. Viscoelastic foam ? Viscoelastic foam is a «slow recovery» foam. During compression, the foam progressively conforms to the shape of the body, and after the weight is removed, the foam slowly reassumes its initial shape. It also reacts to body temperature and ambient temperatures, softening with heat and more easily adjusting to body contours.

How to choose a foam? Density:

Density is expressed in kg/m3, it is the measurement of the mass per unit volume for a cube that is 1m of each side. Foams with higher density have more foam per cubic metre. They are more comfortable, bring a better support and superior resistance to loss of hardness. Density affects : •Quality : the higher the foam density is, the better the foam will retain its original properties. •Price : a higher density means a more expensive price due to higher material content.

Firmness:

IFD (Indentation Force Deflection) : is a measurement of foam firmness. Expressed in Kpa and in Newton (N) : In Kpa (kilo pascal) Express the Force required to compress a foam to 40% •from 1 to 3 kpa = soft •3 to 4.5 kpa = medium firm •4.5 to 6 kpa = firm •> 6 kpa = extra firm In Newton (N): Express the firmness of the foam by indenting (compressing) a foam sample percentage of its original height.

Resilience and Hysteresis :

Resilience is an indicator of the surface elasticity or «springiness». It is measured by dropping a standard steel ball onto the foam from a give height and measuring what percentage the ball rebounds. Hysteresis Loss : Test measures foam resiliency by determining energy loss during loading and unloading. A low hysteresis loss equates to higher resiliency foam. Measured by first indenting the foam sample 25% and measuring firmness, then indenting it 65% and again measuring firmness, and finally releasing indentation to the 25% level without allowing the foam to completely relax. Without fully releasing indentation, foam won’t recover all of its original 25% firmness, but the percentage of firmness it does recover is believed to be a good indicator of overall cushion durability.

Comfort:

Foam’s support factor or «compression modulus» : the hardness at 65% compression (seating pressure) divided by the hardness at 25% (hand pressure) compression. It provides an indication of how supportive the foam will be in use.Support factors range from about 1.5 to 3.5. The higher the number is, the greater the foam’s ability to provide support. A good supportive foam distributes better the weight of the patient. POLYETHER foams reach a max of 2, while High Resiliency foams reach between 2 and 3.5.

Breathability:

Air flow is a measurement of the porosity of openness of foam expressed in cubic feet of air per minute (cfm). Cells deflate when pressure is applied and bounce right back when pressure is released. Foam performance is optimized when air flow is maximized. The more there are broken membranes within the cell structure, the better air flow will be. Air flow affects : •Resilience •Ventilation •Heat feeling
