ABSTRACT There is a need for a change to enhance patient safety in health care organizations. Over this past decade, considerable experience provided valuable insight on existing experiences in the hospital environment with less focus on the primary health care context. This seminar will present first a review of the literature on patient safety in primary health care. Second, we explore the expected adequacy of current primary health care reform and their inherent barriers that may limit the patient safety efforts. Two commonly identified adverse events are well known to be frequent and serious in primary health care: failures or delays in diagnosis and medication prescription errors. According to the literature, better patient safety is associated with wellcoordinated care - a systemic and comprehensive approach overcoming cultural barriers and establishing a safety behavior. Unfortunately, studies showed that efforts through primary care reform have not been sufficient to ensure well coordinated care, so patient safety issues may persist. Patient safety researchers and primary care providers are both aware that a patient safety culture has penetrated primary health care to a much lesser degree than in hospital care. Beginning stages in terms of patient safety awareness and culture needs to be enhanced; this will start by truly implementing well-coordinated care. Cette présentation sera en anglais seulement
